For orders that have multiple ship-to addresses, you can enter this information before you issue the PO by selecting Save to Draft.
Go to the Order Details Page of this draft order, and edit it with a split shipment before you issue the PO. To learn how to split a shipment follow the instructions here.
You can also split the shipment after the purchase order has been issued and include those addresses.
